PROFESSOR ROGER D WEISS

He has focused his research and clinical interests on the evaluation and treatment of 1) patients with cocaine dependence, and 2) substance dependent patients with psychiatric illness. In the 1980s, he was among the first researchers to report the high rate of psychiatric disorders in drug dependent patients. At that time, psychiatrists had little interest in addiction; his work helped to emphasise the importance of psychiatric expertise in treating this population. He next conducted studies relevant to the daily practice of addiction treatment: part of his strength as a researcher has come from his extensive clinical experience as director of the addiction treatment program at McLean Hospital since 1980.
This experience taught the types of questions that clinicians face regularly; his research has thus focused on these issues. For example, he has focused on the process of making psychiatric diagnoses in substance dependent patients (e.g., the utility of various standardised diagnostic instruments, depression rating scales, and biological markers) as well as the importance of recognising heterogeneity within the substance dependent population (e.g., gender differences in cocaine abusers, differences related to the primary drug of choice).
In recent years, his work has evolved from an emphasis on evaluation to a focus on treatment strategies for discrete subgroups of substance dependent and dually diagnosed patients. Most recently, he developed the first treatment designed specifically for patients with coexisting bipolar disorder and substance dependence; pilot-testing has been promising. He is currently testing this treatment both in a randomised controlled trial and in a study designed to test its effectiveness in community-based settings.
He has worked with Dr Lisa Najavits to develop and test a treatment for drug dependent women with post-traumatic stress disorder. As PI of the Harvard site of the NIDA Collaborative Cocaine Treatment Study, the largest controlled study of psychosocial treatment of cocaine dependence ever conducted, he was the first researcher to examine the role of self-help attendance among cocaine dependent patients.
He is currently PI of the Harvard site of the NIAAA-funded Project COMBINE, a multicentre site examining optimal combinations of pharmacotherapy and psychotherapy for alcohol dependent patients. He is also PI for the Harvard site on a NIDA-funded study of Genetics of Cocaine Dependence. Most recently, he became PI for the Harvard University Northern New England Node of the NIDA Clinical Trials Network, a multisite study designed to develop and implement drug abuse treatment studies in community treatment programs.
Finally, he is co-iInvestigator or consultant on other federally funded projects, including the NIMH-funded Systematic Treatment Enhancement Program for Bipolar Disorder, a multisite trial trial examining optimal treatment strategies for patients with bipolar disorder; he is Substance Abuse Pathway Leader for this study.
Funding.
Weiss is currently PI on five federally funded grants. There is a grant funded by the NIDA Behavioral Therapies Development Program to test a treatment he developed for patients with bipolar disorder and substance dependence: Integrated Group Therapy. He is nearing the end of a study comparing that in a controlled trial with Group Drug Counselling. In Integrated Group Therapy, patients focus on both disorders at once, while in Group Drug Counselling, a “parallel” model of treatment takes place, in which the group therapy focuses on their substance abuse and they are told to focus on their bipolar disorder treatment with their psychiatrist. In the study nearing its completion, all therapists conducting Integrated Group Therapy are highly trained professionals with advanced degrees and cognitive-behavioural training. He recently received a grant from NIDA to see if a modified version of this treatment could be delivered by front-line drug counsellors; this is part of an interest on the part of NIDA to increase the likelihood that innovative treatments for drug dependence will be adopted by community drug treatment programmes.
He is PI of the NIAAA-funded Project, “COMBINE,” which compares combinations of pharmacologic and behavioural treatments of alcohol dependence. The study medications are acamprosate, naltrexone and the combination of the two; the behavioural treatments are a brief, HMO-based medical management and a state-of-the-art psychotherapeutic treatment of alcohol dependence.
The fourth project, on which he is Harvard site PI, is Genetics of Cocaine Dependence, funded by NIDA. This project is examining genotypic and phenotypic patterns among families in which two or more siblings are diagnosed with cocaine dependence.
Finally, he is PI of the Harvard University Northern New England Node of the NIDA Clinical Trials Network (CTN). The CTN is a nationwide effort by NIDA to improve the overall quality of drug abuse treatment by forging a partnership between academic research centers and community drug abuse treatment programs. Researchers and community treatment program clinicians work with each other to develop and carry out research studies in community programs.
In addition to these projects, Weiss has an Independent Scientist Award from the National Institute on Drug Abuse on “Treatment of Drug Dependence and Psychiatric Illness. He is co-Investigator on a NIDA-funded study with Dr. Shelly Greenfield, developing a group therapy specifically designed for women, based on recent scientific findings about gender differences in the course of substance use disorder. He was also co-investigator on two studies with Dr. Lisa Najavits, examining group therapy for women with posttraumatic stress disorder and drug dependence, as well as adolescent girls with the same disorders. Although funding for these studies has ended, they are currently analysing data and writing up findings. He is the Substance Abuse Pathway Leader and Co-Investigator for the NIMH-funded multisite trial, “Systematic Treatment Enhancement Program for Bipolar Disorder.” In this project, he examines brief interventions for substance use disorders among patients with bipolar disorder, as they naturalistically occur in clinical practice.
